A federal tax form, copies of which are provided by the employer to employees and to the IRS, stating total earnings; federal, state, and municipal taxes withheld; FICA taxes; and various other deductions including contributions to retirement accounts, unemployment insurance (in some states), and health insurance (in certain states).
