manifest

A one-page form used by transport companies to list the cargo in a shipment. For example, the Environmental Protection Agency requires waste haulers to have a manifest that includes the type and quantity of waste, the generator of the waste, the transporter, and the storage or disposal facility to which the waste is being shipped.
