five whys

A problem-solving technique of asking five times why a particular problem occurred. For example, why does machinery continually break down? The assumption behind this technique is that an average of five whys is required to get at the root of a problem. Of course, five whys is only a rule of thumb, and a solution to a problem may require more or less than five whys. See also Six Sigma.
